SwiftScreen Mobile Drug Testing ("Provider") agrees to provide the following drug testing services to Client:

• Mobile Urine Drug Testing: On-site collection at Client's workplace or designated location
• Testing Panels: 5-panel and 10-panel instant urinalysis tests
• Testing Types: Pre-employment, random, post-accident, reasonable suspicion, return-to-duty
• Chain of Custody: Compliant specimen collection and documentation
• Results Reporting: Preliminary results within minutes; confirmation via certified lab as needed

Client agrees to the following pricing structure:

Payment Terms: Payment is due upon completion of services unless alternative terms are agreed upon in writing. Accepted payment methods: credit card, ACH transfer, check.

Volume Discounts: Custom pricing available for clients with consistent monthly testing volumes exceeding 50 tests/month.

Client authorizes SwiftScreen to perform drug testing services as requested. Client understands that:

• All test results are confidential and will only be released to authorized Client representatives
• SwiftScreen complies with all applicable state and federal privacy laws
• Client is responsible for ensuring proper authorization from employees/candidates prior to testing
• SwiftScreen is not liable for employment decisions made by Client based on test results

Scheduling: Client may schedule testing via phone, email, or online booking at swiftscreen.polsia.app.

Cancellation Policy: Client must provide at least 4 hours' notice for cancellations. Late cancellations or no-shows may incur a $50 service fee.

This Agreement begins on the Effective Date and continues until terminated by either party with 30 days' written notice. Either party may terminate immediately in the event of a material breach.

SwiftScreen's liability under this Agreement is limited to the fees paid for services rendered. SwiftScreen is not liable for any indirect, incidental, or consequential damages arising from the use of testing services.
